POLICIES Minors • No Minors. • Clients must be 18 years old and over to receive services. Informed Consent/ Medical History • Prior to you first visit, you will be asked to fill out and sign a treatment consent form stating that you have read and agree to comply with Luz’n You Up Massage Therapy’s policies and procedures. • A current medical status form is also needed to be completed prior to appointment. • All clients visits and medical/personal information is highly confidential. Booking • Booking confirmation and a reminder will be sent text via text/email 48hours prior to scheduled appointment time. • Please confirm, reschedule or cancel as needed. Late Arrival Policy • To ensure a full massage session, please arrive at least 10-5 minutes before your scheduled appointment time. If you're running late, please call as soon as possible. • Due to scheduling, clients who arrive 15 minutes after their scheduled start time may have session shorten by ending at the normal time out of fairness to the following clients. • The original session cost will still be charged. Cancellation Policy/ No Show Policy • Scheduled appointments must be canceled or rescheduled 24 hours in advance to avoid cancellation fees. • Please provide at least 2 hours notice if you need to reschedule or cancel an appointment. • If a client fails to cancel 2 hours before scheduled appointment time, 100% will be charged. • Appointments canceled within the 2 hours scheduled appointment, will be charged 50%. • To cancel or reschedule please call at (757) 603-3803 or online at luznyouup.glossgenius.com Insurance Billing • Luz’n You Up Massage Therapy does not offer direct billing to insurance companies. An itemized receipt can be provided that you may submit to your insurance.
